When receiving care at Somerset Medical Center, you will need to provide the following information to our registration team:

• Your name
• Date of birth
• Social Security Number
• Address
• Telephone
• Insurance coverage

This information, which is handled confidentially, ensures that accurate data is provided to those involved in your care.

If you are visiting the medical center for a specific procedure or testing, please contact the department in which you are having your procedure to schedule an appointment before you register. You may consult your physician for the name and number of the department in the medical center, or visit Our Services.

Pre-Registration
Somerset Medical Center can pre-register your information in our system to save you time during the day of your visit.

• Call 908-704-3745, and a member of our registration team will confidentially take your personal and insurance information over the phone. Please allow at least three days prior to your visit and have the date of your appointment and your insurance information ready when you call.
  
• If your visit is at least 3 business days away, click here to pre-register your information through our secure Web site.

Pre-Admission Center
If you are having a same day surgery procedure or being admitted, you may need to complete Pre-Admission Testing (PAT), which can include tests such as lab work, an EKG or a chest X-ray. If you are a same day surgery patient, please call Pre-Admission Testing at 908-685-2918 to register and make an appointment.

When you Arrive
On the day of your visit, please be sure that you have at least one form of identification, your insurance card and any other documents that you will need such as referrals, pre-authorization numbers, and/or physician orders. Also, please note that your insurance plan may require that a co-pay be paid at the time of service. Please report directly to the department where you will be having your procedure. Cardiology patients having procedures performed on the weekend should report to registration.
