Patients should call the department to schedule an appointment.
Patients should bring insurance card, prescription, list of current medications,
insurance referral (if needed) and drivers license.

Rx/Requirements: A dated prescription from the doctor stating
diagnosis, frequency and duration of treatment is required.

Reporting Results:
Speech, Language, Cognitive Evaluation reports are faxed to physicians within 1 week. Swallowing Evaluations/MBS
reports are faxed to physician within 12 hours.
Please note:
Evaluation and therapy services can be provided for speech, language,
cognitive, swallowing and voice disorders. We perform MBS, Video
Stroboscopy, FEES and VitalStim therapy. Services available
for children and adults. Audiology services also are available
onsite through Brian McClintock, audiologist. These services are not
billed through the hospital.
