Outpatient Instructions: Patients should call the
department to schedule an appointment. Patients should bring insurance card,
prescription, list of current medications, insurance referral (if needed) and
drivers license and should wear comfortable clothing.
Any
Rx/Requirements: A dated prescription from the doctor stating
diagnosis, frequency and duration of treatment is required.

Reporting Results: Copies of all
evaluations and progress notes are faxed to the referring physician within 24 to
48 hours. Re-evaluations are performed at least every 4 to 6 weeks and are faxed
to maintain open communication between physician and therapist.
Please note: Care is provided for a
variety of diagnoses, including orthopedics, neuro disease, stroke
(NDT-certified therapist), balance and vestibular issues and lymphedema.
Department features McKenzie therapist and tai chi program to improve balance
and endurance for oncology patients. Appropriate for all patients ages 12 and
above. Free parking for rehab patients. |
